The short answer

Fill in Teachers Mutual Bank's Name Change Request Application form with your member number and your name now and new. Give two of your driver licence, passport or Medicare card in your new name so they can verify you electronically, no certified certificate needed if these match. Sign it, then email it to mso@tmbl.com.au or post it to Teachers Mutual Bank Limited, GPO Box 5313, Sydney NSW 2001.

On the Powered plan, Surname Switch fills the whole form in, signature included, and asks the few things only you can answer.

1 One form

One form, sent your way

Teachers Mutual Bank has no online self-service for a name change. Every member uses the same Name Change Request Application form, and the same form covers UniBank, Firefighters Mutual Bank, Health Professionals Bank and Australian Mutual Bank too.

Once it is filled in and signed, email it to mso@tmbl.com.au or post it to Teachers Mutual Bank Limited, GPO Box 5313, Sydney NSW 2001.

2 The form

We fill your form in for you

The Name Change Request Application form asks for your member number, your name now and your new name, your address, phone and email, your date of birth, your occupation and your country of birth.

On the Powered plan, Surname Switch fills every one of those in from the details you enter once. Your signature is stamped, with today's date. The form comes back ready to sign, if you have not already, and send. Your signature is used in the moment and never kept.

3 No certificate needed

Verified electronically, not by certificate

Give two of your driver licence, passport or Medicare card, each in your new name, and consent to an electronic check against a credit reporting body and government records. If they match, that is it: no certified copy of your marriage certificate, change of name certificate or deed poll needs to be attached.

  • Which two IDs: your choice of any two, asked plainly, never assumed.
  • Consent: say yes for the electronic check, or no and Teachers Mutual Bank contacts you to verify another way.
4 Only what's yours

Only tick the products you hold

The form also lists a credit card, a Visa debit card, PayID, safe custody and Zurich Ezicover life or income protection insurance. Every one of these varies from member to member, so each is asked plainly and ticked only when you say yes. Nothing is assumed from the fact that you bank with Teachers Mutual Bank.

Where Teachers Mutual Bank fits in your name change

Your bank sits early in the money phase of your plan, usually alongside your super fund and any share trading account. If your bank is Commonwealth Bank, see our CommBank guide. Our bank-by-bank comparison covers the rest, including AMP Bank in our AMP Bank guide and nabtrade for share trading in our nabtrade guide.

Married in Australia? Your marriage certificate is proof, no separate legal step (see our marriage guide). Choosing a brand new name? See our deed poll guide. Going back to a former name? See our divorce guide.

Remember, even without a certificate to attach, the electronic ID check still needs your driver licence, passport or Medicare card to already show your new name.

Teachers Mutual Bank is one stop of many. Surname Switch builds one ordered checklist with your real bank, super fund and investment platform. It fills forms where one exists, and drafts AI messages elsewhere.

Your details stay on your device, never our servers. See what each plan includes, or the full checklist in our name change checklist.
